Any student athlete that has been added to a team roster by the head coach is required to complete and submit all of the appropriate medical forms for clearance. If you have not met with the coaching staff regarding team participation for the upcoming sports season, please do so before submitting your medical packet. All medical packets are linked below for NEW and RETURNING athletes.
If you are covered under your family's medical insurance, then it is advised that you see your primary care physician to complete your physical paperwork. If you don't have a primary physician, then we have provided a list of Stockton Medical Facilities linked below.
